ATTENTION: Please be advised that on Monday, August 18th, 2025, our vendor will be performing a data import from the production environment into the CHIRP QA environment. As a result, the CHIRP QA environment will be unavailable from 1 PM until midnight on this day.

• To access CHIRP QA following the import, you will need to log in using your CHIRP Production credentials. If you do not recall your production password, please utilize the "Forgot Password" link on the login page to reset it.
• If you do not have a production log in, please download and fill in the Individual User Agreement form located on the CHIRP home page and ask to CHIRP QA access. Email the form to chirpaccess@health.in.gov.

Individual User Agreement - School Nurse form will be replaced with an online version. Beginning September 9th, all School Nurse User access submissions must be completed online. After this date, paper versions will no longer be accepted or supported.

Attention CHIRP Users:

Please be aware that, per the Individual User Agreement (IUA) policy, CHIRP user accounts are automatically inactivated after 90 days without a login.

To reactivate your CHIRP account, please submit a new IUA form using this link. If you have any questions or need assistance, please contact the Help Desk at 1-888-227-4439.

Attention School Nurses!

CHIRP Help Desk is available Monday through Friday from 8:30 AM to 5 PM EST with a lunch break between 12:30 PM and 1:00 PM. The Help Desk is closed weekends and on all Indiana State holidays, CHIRP website is available 24 hours a day, 7 days a week.
